Insurance Providers

Blue Mesa Medical Associates accepts most major medical insurance EXCEPT the following:

• Tricare
• Secure Horizons
• HMO Blue
• Cigna Select
• AMCARE, Medicaid HMOs and NHIC Medicaid

Please check your insurance provider list or call or email us to see if we take your particular insurance.

In addition, we welcome cash pay patients who have no insurance.

We provide services for Workers' Compensation and disability rating.

Unless other arrangements have been made in advance, Blue Mesa Medical Associates expects payments at the time services are rendered.  If we are unable to verify your insurance, you will be expected to pay by cash or credit card for your office visit and any necessary testing.  We will be happy to provide you with an itemized receipt so you can file a claim for reimbursement from your insurance provider.
