Practitioners
- In an effort to improve the practitioners' efficiency, we ask that email be sent to the practitioners only if your question cannot be answered by anyone else.
- DO NOT email if you are sick, please telephone instead.
- Send prescription refill and test result requests to the nurse.
- If your question can be answered by someone else, it will be forwarded at the discretion of the provider.

Insurance
- For all insurance and referral questions except in regards to billing for services already rendered, please email Referrals.
Billing and Office Management
- Please send general office questions, complaints, suggestions, etc., or billing questions to Office Manager.
Medical Records
- Requests for medical record transfers or copies can be sent to Medical Records
- Please note: We will require a signed medical records release. Please include a fax number with your request so that we may fax you a release form. Allow five business days after we receive your signed release for records to be transferred or copied.
